Nurse Aide and Medication Aide Guidelines

Review the guidelines below prior to submitting the online renewal forms:

Submit separate renewal forms for listings on the Nurse Aide I Registry and the Medication Aide Registry for Nursing Homes.
Submit the renewal forms within 90 days before the registry listing expiration date.
The last date worked must fall within the registration/listing period identified on the renewal form.
