Renewal

Registry listings must be renewed every 24 months. Nurse Aides are responsible for knowing when their listing expires. The Division of Health Service Regulation does not send renewal notifications.

To qualify for renewal, a Nurse Aide must meet all of the following requirements. Nurse Aides who do not meet these requirements must complete retraining and retesting.

A registry listing cannot be renewed if the Nurse Aide has a substantiated finding of abuse, neglect, exploitation, or misappropriation of resident or patient property while employed in a nursing home. 

To renew a registry listing, the following forms must be completed:

The registry expiration date is based on the last date of qualified work reported by the Registered Nurse in the Online Employment Verification Form. Expiration dates are extended to the last day of the month.
